All Athletes must complete the new NEISD Electronic Athletic Participation Form (Click here). Select High School. Click on Athletic Participation Packet (High School). Fill in all required fields and click Submit. A copy of the completed form will be e-mailed to you. Under Printable Forms, download the Medical History and Physical Form to take to your physician. Once complete, return the form to your Coach or Athletic Trainer.
