All incoming 9th – 12th grade students are required to complete the NEISD Athletic Participation Form and Medical History Form in order to participate in Reagan Athletics for the 2020-2021 school year. CLICK HERE or go to https://northeastisd.rankonesport.com/New/NewFormList.aspx. Please note both student and parent signatures are needed on both of these on-line forms. Once completed, your form will show “Pending Approval”. Once approved, it will be available to download a pdf copy for your files.
“Yes” answers to certain questions on the Medical History Form will require a student to receive further medical evaluation before any participation in UIL practices, games, performances, or matches.
